Background: The aim of this study was to perform translation, cultural adaptation, and validation of the Glasgow Coma Scale (GCS) and the Full Outline of Unresponsiveness (FOUR) scale in Russia, as well as to assess their psychometric properties. Methods: The original versions of the GCS and the FOUR scales were translated into Russian by two independent interpreters and then back-translated into English. The resulting version considering linguistic features was approved for use by the Expert Commission. Then, taking into account inclusion and exclusion criteria, a pilot test was performed by assessing 15 patients with the developed versions. Based on the results, the final versions of the scales were adopted. As part of the second stage of the validation study, two researchers (neurology residents) prospectively evaluate patients with the GCS and the FOUR scale in a neurological critical care unit. Onthefirstday, rater1 and2 independently scored each patient, the comparison of their evaluations was used to assess inter-rater reliability. In 15 minutes break rater 1 had to make a second evaluation, by this test-retest reliability was measured. In 24 hours observer 1 had to terminate the third evaluation. The comparison of 1stand 3rdassessment by the same person was used to define the sensitivity to changes in the state of patients with acute disorders of consciousness. �Results: As a result of the completed phase of linguistic and cultural ratification we have Russian versions of the GCS and the FOUR scale, that are adopted for use in Russia and other Russian-speaking countries. As for the evaluation of psychometric properties 35 patients with acute disorders of consciousness of various origin have gone through the assessment using developed versions of both scales. In total 140 evaluations have been made with developed versions by two researchers. Discussion and conclusions: The Russian versions of the GCS and the FOUR scales are available for use in clinical and research practice. Further patient recruitment is going on in order to evaluate psychometric properties and confirm by statistics validity of our versions. �
